Appointments

     Mon - Fri:      7 am - 9 pm

     Saturday:      8 am - 5 pm

 

All imaging exams require a referral from your physician. Once we have received your doctor's order by phone or fax, you can call our office to schedule your exam. Every effort will be made to accommodate emergency or add-on requests. Walk-in patients are welcome.

 

When calling for an appointment, please be prepared to provide the following information about the person for whom the exam is being ordered:

  • Name

  • Address

  • Social security number

  • Insurance information

  • Location of relevant imaging studies/films done in the last 2 years

  • Date of follow-up appointment with referring physician

Depending on the exam, you may be asked other questions to ensure your safety and the appropriateness of the procedure. These issues are covered under the Exam Guidelines for each modality in the About Your Exam section of this site. In addition to the information available there, you will be mailed all pertinent information about your exam so you will know what preparation is needed, if any, and what to expect on the day of your appointment.

 

Insurance Issues

Edmond 3T Imaging Center and Clinics accepts most major insurance plans.  We’ll make every effort to contact your insurance company to gain authorization or certification for the ordered exam and verify coverage.  However, you are responsible for paying for services, including any balance not covered by insurance.  (See the Insurance and Billing section of this site for more details and listing of contracted providers.)

 

Required Patient Forms

For your convenience, you can print out and complete some of the required paperwork in advance of your appointment.  All patients must complete the Patient Registration and Patient History forms.  Depending on your exam, other forms may be required.  Go to the Patient Forms section of this site and click on the appropriate links.

 

Exam Day Check-In

On the day of your exam, we ask that you arrive 30 minutes prior to your exam time.  This will allow us to verify your registration information and have you fill out any additional forms, as needed.  Be sure to bring your insurance card(s) and any of the patient forms you printed out and completed.

 

Patients whose procedure requires an oral contrast for imaging enhancement will need to arrive an hour ahead of the exam to drink the contrast.  If this pertains to your study, you will be advised of this requirement in your Patient Preparations guidelines.

 

If you are unable to keep your appointment, please notify our office at least 24 hours in advance, or as soon as possible.

 

Reports

Reports are normally sent to the referring clinician within 24 hours of the exam.  The imaging study done at Edmond 3T Imaging Center and Clinics may be one tool in your clinician’s work up.  Thus, they may need additional time to correlate all of the associated reports and data before they are ready to talk to you.  Edmond 3T Imaging Center and your referring clinician politely requests that you wait for your clinician’s office to contact you about the scan results.

 

Film Reprint Policy

Referring clinician offices will be provided image on a CD or via the Internet(HIPPAA complaint).

 

Clinician offices that require film of the imaging study will be provided at a timely fashion. If additional copies of the study are required for a second referral or a personal copy, we will be happy to provide a CD copy of your study at no charge.

 

Note:   If you want to have a CD sent to an address other than your home address we have on file, or to your clinician of record, or to be picked up by someone other than yourself, we will need to have a consent form signed by you.
